Services

Community Support Services assist consumers in identifying, securing, and sustaining the range of resources, both external and internal, needed to live in a normal, interdependent way in the community. The consumer identifies goals in the areas of daily living, financial, vocational/educational, social supports, health, leisure, and spiritual supports. The case manager functions as the primary clinician and as thus, coordinates all needed services including medical, pharmacological management, psychosocial, psychotherapy, substance abuse, and crisis services. Services are targeted to adults in Shawnee County who have a severe and persisitent mental illness (SPMI) , may have a co-occurring substance use disorder, and/or are Psychiatric Rehabilitation Eligible (PRS).

  • Special Populations Accommodations (P.A.T.H., Choice Team, H.O.P.E. Connection, and Tanglewood)
  • Expressive Therapies- Art therapy, groups, and rehabilitation
